SUMMARY: Responsible to the Fatherhood/Transitional Jobs Program Manager for the provision of program services to individuals participating in the Transitional Jobs program which assist participants in developing employability skills, addressing personal barriers to employment, obtaining and maintaining employment. Primary functions of the position include partnering with clients to: assess strengths, needs and goals; develop an individualized plan for meeting needs and achieving goals; refer and link to community resource providers; assist with the job readiness skills; determine appropriate employment placement; monitor and report on participant progress in employment placement and address issues with employers.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
· Perform crisis management and other related counseling activities to maintain participants at the employer worksite which will require availability and flexibility to meet with workers and employers during various production shifts as necessary.
· Maintain attendance records of participants, monitor and review accuracy with Fatherhood/Transitional Jobs Program Manager.
· Coordinate with CAI staff in career assessments, workshops, and development of community business relationships for potential job sites.
· Coordinate with employer relations staff to conduct job analysis of potential client job duties and responsibilities; Liaison between client, employer relations staff, and community employer at time of hire, job orientation.
· Formulate and revise participant Individual Employment Plans, incorporating participant needs and preferences, measurable goals and objectives, and appropriate employment environment.
· Ensure that needed services are provided either directly by CAI staff or by referral to appropriate support service agencies.
· Monitor Individual Employment Plans and report outcomes to ensure the ongoing provision of quality services. Conduct Individual Employment Plan staffing as necessary, for periodic review of individual participant needs and effective goal setting.
· Maintain case notes and participant plans in CARES system. Assist participants with onsite job training.
· Compile and maintain active and inactive case records in a confidential fashion as prescribed.
· Prepare, distribute and maintain files of all written correspondence, communications and/or other reports as designated by administrative staff.
· Completes all required documentation relative to employer and funding sources.
· Maintain compliance with relevant regulatory agencies.
· Monitor, correct, and report unsafe working conditions and/or behaviors.
- Authorize and process direct financial assistance related to client services, as resources allow.
- Maintain effective communications with all internal and external customers

NON ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
- Complete CARES training and utilize as the primary system to maintain participant information.
- Attend staff, department, agency and local community meetings as required
- Complete related work as assigned within the scope of responsibility and skills required for the job
- Follow-up on all program service referrals/requests

EDUCATION AND EXPERIENCE:
- Two years equivalent combination of education and work experience involving diverse populations, corrections, or non-custodial/non-residential dads is required.
LANGUAGE SKILLS:
· Requires excellent listening, verbal and written communication skills, with strong attention to detail and accuracy.
MATHEMATICAL SKILLS:
· Knowledge of basic math: add, subtract, multiply, divide, fractions and percentages.
REASONING ABILITY:
· Ability to interpret a variety of instructions in written, oral or diagram form, to solve problems requiring decision-making skills within department guidelines.
CERTIFICATES, LICENSES, REGISTRATIONS:
· Access to reliable, licensed,insured driver and transportation required.
OTHER SKILLS AND ABILITIES:
- Excellent interpersonal skills with nonjudgmental approach and style.
- Ability to work effectively in a team service delivery environment.
- Ability to work effectively with a diverse group of program participants.
- Ability to manage multiple projects and meet deadlines.
